NTSB Blames Indiana Pilot In Fatal Crash
Pilot Flying Too Low, Preflight Planning Inadequate, Report Says

CHICAGO -- The National Transportation Safety Board's final report on a 2008 helicopter crash involving an Indiana pilot in which four people died places blame primarily on the pilot.Del Waugh, 69, of Carmel, was flying too low and his preflight planning was inadequate, the NTSB said.

Waugh was piloting a Bell 222 helicopter when it struck a radio station tower guy wire in Aurora, Ill., on Oct. 5, 2008.The report also indicated that an air traffic controller contributed to the crash by failing to issue a safety alert.Waugh, along with a 14-month-old patient, a nurse and a paramedic died in the crash as the Air Angels Inc. flight transported the patient to a Chicago hospital.Air Angels went out of business in February 2009 after the deceased child's parents sued.
