Mother says son's headstone not placed at cemetery five months after his death

INDIANAPOLIS -- An Indianapolis mother says New Crown Cemetery still has not placed her son's headstone five months after his death.

Amy Smith says she was told the stone was ordered in May but it was not.

It has since been delivered to the cemetery however, it has not been laid out on her son's grave.

New Crown Cemetery acknowledges there may have been some issues with ordering the headstone.

Cemetery management released this statement:

The headstone for Nicholas Smith was paid for on March 11, 2016 and processed on March 14, 2016. After a headstone is purchased the family needs to come in and design how they would like the headstone to look. On May 24, 2016, Amy Smith came out and designed the stone and signed a proof (computer generated rendering) which then places it into production. The stone arrived on July 12, 2016 and a work order was created; the headstone is currently awaiting installation. Updating cemetery processes and procedures is and always  will be perpetual. Our goal is to serve the community and give people an affordable place to lay their loved ones to rest. We will continue striving for perfection and taking all things into consideration when reviewing the best way to help reduce the anxiety, expense, and inconvenience when dealing with the loss of a loved one.

Cemetery representatives say they had a record number of services and burials last month, but they did get the stone placed Monday night.
