- Type of organization (school, day care/preschool, business, government, church, or other -- specify)
- Organization name, address (including city, ZIP code and county) and phone number
- Contact person's name, business email address and after-hours phone number
- No. of employees
- No. of others affected by closing
During periods of high volume, requests for new accounts will take up to 72 hours to process.
